Observation

WAC 110-300-0345(3)(d) - Staff were not attending to a child's activities and were not aware of what the child was doing at all times as required. This is evidenced by the fact that a child had a minor injury during child care and the staff persons working with the child that morning have no knowledge of how the injury occurred.
